When you submit a form we collect your name, phone number, email address (optional), the treatment you are interested in and the message you write. We also record which page you were on and how you arrived at the site, so we can understand how patients find us.
What we do not collect on this site
We do not ask for, and ask you not to send, medical history, insurance member IDs, photos of insurance cards or any other protected health information through this website. That information is collected securely by phone or in person.
